About the Role:

As an Evening Shift LPN at Haven Place, you'll be responsible for providing direct patient care, including:

• Administering medications and treatments as ordered by physicians

• Assisting with activities of daily living, such as bathing, dressing, and grooming

• Monitoring vital signs and reporting changes to the healthcare team

• Providing emotional support and companionship to our residents

• Collaborating with the interdisciplinary care team to develop and implement individualized care plans

Responsibilities:

• Provide direct patient care under the supervision of RNs and other healthcare professionals

• Conduct vitals checks, lab tests, and other medical procedures as needed

• Document patient information accurately and thoroughly

• Participate in quality improvement initiatives to enhance patient outcomes

• Maintain a safe and organized work environment, adhering to all infection control guidelines

Requirements:

• Current LPN license in the state of Pennsylvania
