Key Responsibilities\n

• Build rapid rapport with clients and engage in meaningful conversations to facilitate their recovery journey, utilizing active listening and a non-judgmental approach.

• Collaborate seamlessly with a diverse team of healthcare providers, including behavioral health professionals and primary care physicians, to ensure holistic treatment of patients.

• Conduct evidence-based and developmentally appropriate screenings to assess mental health conditions, substance use issues, and other behaviors that may impact overall health, while coordinating further assessments as needed.

• Develop and manage a caseload of patients, providing ongoing brief interventions and follow-ups tailored to individual needs.

• Utilize the electronic medical record system effectively to document patient encounters and treatment plans, ensuring timely updates within 48 hours.

• Create integrated care plans that link patients to necessary services, ensuring seamless communication among all parties involved in the patient’s care.

• Uphold patient privacy and confidentiality standards in all communications and documentation.
\n
• Apply principles of population-based care to advocate for preventive and chronic care strategies that enhance patient wellness.

• Actively participate in mentorship and professional development through supervision sessions, case conferences, and quality improvement meetings.

• Facilitate referrals for patients needing psychiatric evaluations when indicated and maintain open lines of communication with patients and families.

• Identify and guide patients towards relevant community resources, aiding in their recovery beyond clinical support.

• Carry out additional responsibilities as required to enhance patient care and support the detox facility.

Qualifications\n
