Key Responsibilities\n

• Research, refer, and advocate for essential community resources, including food assistance programs, childcare services, and healthcare access to enhance clients’ overall well-being and ensure continuity of care.

• Act swiftly in crisis situations, such as child abuse allegations and mental health emergencies, providing immediate support and solutions to safeguard clients' welfare.

• Develop individualized treatment plans, regularly monitor client progress, and adjust strategies to align with their evolving needs while celebrating milestones and successes.

• Engage with clients, offering guidance in adjusting to changes in life circumstances such as illness, divorce, or job loss, and helping them navigate through these challenges thoughtfully.

• Maintain accurate records, document patient interactions, and comply with the Department of Mental Health billing and paperwork requirements to ensure accountability and funding integrity.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.
