Key Responsibilities\n

• Conduct in-depth mental health assessments including psychosocial evaluations, diagnostic interviews, and thorough risk assessments to understand patient needs.

• Develop customized treatment plans in collaboration with patients, considering their specific mental health challenges, goals, and cultural backgrounds.

• Implement evidence-based therapeutic interventions such as cognitive-behavioral therapy (CBT), dialectical behavior therapy (DBT), and solution-focused brief therapy (SFBT) tailored to individual, couple, family, and group dynamics.

• Provide immediate crisis intervention services, collaborating with team members across disciplines to ensure continuity of care for patients experiencing acute distress.

• Maintain meticulous clinical documentation, including treatment plans, progress notes, and discharge summaries, adhering to healthcare regulations and clinic policies.

• Educate patients and their families about mental health conditions, empowering them with the knowledge and skills necessary for effective management of their emotional well-being.

• Work closely with a multidisciplinary team, including psychiatrists, psychologists, nurses, and case managers, to tailor comprehensive treatment strategies for our patients.

• Stay updated on the latest research and methodologies in mental health care, integrating this knowledge into your practice to enhance patient outcomes.

Patient Population / Client Focus\n

As a Licensed Clinical Social Worker in our hospice center, you will primarily work with patients facing terminal illnesses and their families. Compensation and Benefits\n
