Work Environment

The role of the Full-Time Physical Therapist is primarily onsite within the homes of our clients. You will have the autonomy to create personalized treatment plans and adjustments based on each patient's unique needs and environments. Key Responsibilities

• Conduct comprehensive evaluations of patients, setting measurable rehabilitation goals, and developing individualized treatment plans in accordance with physician orders.

• Implement therapeutic interventions that are tailored to the patient's age, functional status, and health conditions, while continually assessing progress towards set objectives.

• Maintain accurate and thorough documentation of patient interactions, treatment plans, and progress notes using professional medical terminology that substantiates the clinical necessity for services provided.

• Provide guidance and monitoring for Physical Therapy Assistants (PTAs) and support personnel, ensuring they are effectively contributing to the patient's care and their outcomes.

• Collaborate with a multidisciplinary team, including physicians, occupational therapists, speech-language pathologists, and nursing staff to ensure cohesive care delivery.

• Actively participate in team meetings and engage in professional development opportunities to enhance clinical skills and stay abreast of new treatment methodologies.

Patient Population / Client Focus

As a Physical Therapist at Guided Grace Therapy Center, you will primarily serve a diverse patient population requiring rehabilitation due to various conditions, including orthopedic injuries, neurological disorders, post-operative care, and chronic pain management. You will work with patients of all ages, customizing your approach to suit the developmental and rehabilitation needs of each individual, thereby ensuring a significant impact on their rehabilitation journey.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited university.

• Current licensure as a Physical Therapist in the state of Virginia.

• Effective verbal and written communication skills to foster strong relationships with patients, families, and interdisciplinary team members.

• Proficiency in using electronic health record (EHR) systems for patient documentation and care management.

Preferred Experience

• A minimum of 2 years of experience in a physical therapy setting, preferably within home health care.
