Access Coordinator

Summary/objective To support individuals seeking services from OnPoint through phone and in-person contact focused on providing and obtaining the information necessary to initiate and advance the access process.

Essential functions:

• Respond to phone inquiries from individuals seeking information about access to services.
• Function as the first point of contact for those who come to the agency seeking services through the same-day access process or scheduled appointment.
• Provide an overview of the access and intake process and answer any questions individuals may have.
• Engage with individuals to obtain demographic information and an overview of the reason for seeking services.
• Obtain insurance and financial information, explaining the rationale and ability to pay (ATP) process.
• Provide insurance and financial information to identified reimbursement specialist.
• Review and complete required consent forms.
• Support individuals to complete access documents and forms.
• Ensure care coordination focus.
• Complete documentation according to established accreditation, regulatory, funding, and best practice standards.
• Perform other duties as assigned.

PAY RANGE/BENEFIT PACKAGE: Salary: starting at $57,948.80 up to $79,955.20 annually - placement above minimum salary is based on experience. OnPoint Benefits:

Environment (Work, Physical, Travel):

• Work Environment – Environments can vary. Primarily work in an office environment, but travel by automobile to other locations within and outside of the County may be required.
• Physical Demands – Generally, performs sedentary work but must have the ability to lift up to 35 pounds. Occasional bending, stooping, kneeling, lifting. Ability to sit for extended periods of time.
• Work a schedule of hours to accommodate client need, including evenings.

Required education and experience:

• Bachelor's degree in human services field
• A minimum of one-year relevant work experience with the target population

Preferred education and experience:

• Bachelor's degree in social work
• Lived experiences with mental illness/developmental disabilities/co-occurring disorders/substance use disorders.
• Experience in the public behavioral health services

Additional eligibility requirements:

• Must possess a valid driver's license, registration and proof of automobile insurance when operating County vehicles or operating personal vehicles in performance of job duties at any time. May provide own transportation for job related use, including transporting of individuals served, to and from meetings and activities at varying work locations throughout Allegan County.
