- Conduct thorough evaluations for patients in both inpatient and outpatient settings, tailoring treatment plans for their specific needs
- Implement therapeutic interventions aimed at enhancing mobility, improving strength, balance, and overall function
- Collaborate closely with nursing staff, physicians, case managers, and fellow rehabilitation specialists to achieve optimal patient outcomes
- Provide educational support to patients and their families regarding therapeutic objectives, discharge procedures, and at-home exercise routines
- Ensure detailed documentation of evaluations, treatments, and progress, adhering to facility and regulatory standards
- Engage in interdisciplinary conferences and care team rounds for comprehensive and coordinated patient care
- A Doctorate in Physical Therapy (DPT) or a similar credential from an accredited institution
- State licensure as a Physical Therapist
- CPR certification preferred, with specialized certifications (such as in neurology or geriatrics) seen as a plus, though not essential
- Prior experience in a similar role is highly preferred
- A comfort level in treating patients with complex medical and mobility challenges
- Strong collaborative skills and excellent communication abilities
- Familiarity with digital charting and clinical documentation is highly desirable
