Key Responsibilities\n

• Identify and recommend the type, length, and intensity of services required to address the unique needs of clients.

• Deliver virtual support services through tele-presentation and telephone consultations, ensuring consistent access to care.

• Coordinate with primary care providers, specialists, and home health aides to streamline client care transitions and maintain continuity.

• Manage comprehensive case management for clients, ensuring that all aspects of their well-being are addressed.

• Engage families and caregivers in the therapeutic process by providing resources and support to enhance their involvement in client care.

Your work will primarily focus on adults who may be experiencing a variety of mental health challenges. Qualifications\n

• A Bachelor’s degree in Social Work (BSW) and an unrestricted state license as a Licensed Clinical Social Worker (LICSW).

Compensation and Benefits\n
