This opportunity offers a challenging 13-week travel assignment as a Staff Pharmacist in a Long-Term Acute Care (LTAC) hospital setting. You'll play a vital role in ensuring safe, effective medication use and contributing to excellent patient outcomes. Key Responsibilities:

• Maintain appropriate drug inventory, oversee procurement, and manage emergency medication supplies.
• Prepare, sterilize, and label medications including parenteral solutions in compliance with hospital protocols.
• Dispense medications accurately by interpreting drug orders and reviewing work from support staff.
• Conduct regular inspections of pharmacy and patient care areas, ensuring cleanliness, safety, and regulatory compliance.
• Oversee controlled substance management, documentation, and distribution.
• Participate actively in Pharmacy and Therapeutics Committee meetings and compliance initiatives.
• Provide patient education, discharge counseling, and medication reconciliation leveraging EPIC system expertise.
• Support antimicrobial stewardship by interpreting culture results and making clinical recommendations.
• Monitor pharmacokinetic dosing and anticoagulation therapy with precision.
• Develop and lead pharmacy staff training and continuing education programs.
• Collaborate with multidisciplinary teams to optimize pharmacotherapy and quality assurance processes.

Qualifications and Desired Experience:

• Prior hospital or LTAC pharmacy experience required.
• Active BLS certification.
• Proficiency with EPIC electronic medical records system.
• Strong clinical knowledge of antimicrobial stewardship, pharmacokinetic dosing, and anticoagulation management.
• Excellent communication skills for medication history gathering and patient/family discharge counseling.
