Social Worker III- Evening Shift

Position Summary At Stony Brook Medicine, Social Workers provide empathic care to patients, families, and visitors with a high degree of integrity and respect. They apply professional principles to deliver integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families, and groups across various settings. Social Workers plan, implement, coordinate, and document treatment for individuals diagnosed with mental illness, serious emotional disturbance, developmental disabilities, work‑related illnesses, chemical addiction, and other conditions requiring therapeutic intervention. Duties Psychosocial assessment of high risk cases. Effectuate discharge planning. Coordinate with medical teams, community partners, and transitional care teams. Charting and documentation including training, data management, and statistics. Assist patient financial services, including Medicaid and other entitlement programs. Serve as liaison between community agencies and hospital. Prepare material for guardianship, oversight agencies, and surrogate decision making. Prepare weekly updates for the Long Stay Committee. Lead therapeutic groups when appropriate. Supervise professional social work students when appropriate. Participate in performance improvement activities, program development, data collection, and analysis. All other duties as assigned. Qualifications Required: MSW from an accredited program, NYS license within one year of hire, experience in medical health care, excellent written and oral communication skills, and proficiency in discharge planning and long‑stay barriers. Must be computer literate and able to implement and run medical team rounds, manage large complex case loads, and demonstrate patient‑first advocacy. Preferred: Experience in multiple medical specialties, inpatient or skilled nursing settings, discharge planning, diagnosis‑specific social work, and bilingual ability. Prior to Start Date Requirements Complete a pre‑employment physical examination and obtain medical clearance. Complete electronic reference check with at least three professional references. Pass a 4‑panel drug screen. Meet regulatory requirements for pre‑employment screenings. Provide copy of any required New York State license(s) or certificate(s). Benefits The salary range for this position is $86,162 - $109,275 base, plus up to $4,000 location pay and up to $10,000 in shift differential. Additional benefits include generous leave, health plans, and a state pension.
