DESCRIPTION:
Conduct client assessments, develop and monitor care plans, coordinate community-based services, maintain regular client contact, complete required documents and monitor client service delivery. Must have social service background and computer skills. Will perform other duties assigned. Must have a valid Texas Driver’s license and comply with AP 2-2 policy.
MINIMUM QUALIFICATIONS     
KNOWLEDGE: Requires a Bachelor's degree in a social or behavioral science, such as Sociology, Psychology, Social Work or Counseling.

EXPERIENCE : Three years of professional experience in counseling or related social services work are required. A Master's degree in a social or behavioral science may substitute for two years of the experience requirement.

PHYSICAL EFFORT: The position is physically comfortable; the individual has discretion about walking, standing, etc.

WORK ENVIRONMENT: There are no major sources of discomfort, i.e., essentially normal office environment with acceptable lighting, temperature and air conditions.

PHYSICAL SKILL: Requires the ability to make simple gross motor responses within large tolerances.
