Key Responsibilities\n

• Conduct thorough assessments to deeply understand and address patients' needs and treatment goals.

• Establish effective communication channels with supervisors, family members, and other caregivers by providing regular updates on patient progress.

• Develop and present tentative schedules for appointments, home visits, and monitoring strategies to ensure all client needs are met.
\n
• Coordinate home visits that will include chart reviews, staff interviews, and data collection to ensure comprehensive assessments.

• Create, implement, and monitor personalized treatment plans that meet each patient’s unique challenges and objectives.
\n
• Participate actively in Interdisciplinary Team Meetings, providing insight into treatment updates and collaborating with fellow healthcare professionals.

• Access and update electronic medical records (EMR) to maintain accurate documentation of patient interactions and therapy progress.
\n
• Lead training sessions to ensure staff members understand and adhere to program plans and documentation protocols.
\n
• Conduct follow-up evaluations to track patient progress and maintain engagement in treatment.
\n
• Serve as a liaison between the clinic and the wider mental health community to foster relationships that support treatment goals.

Patient Population / Client Focus\n

You will work with a diverse clientele, including adults, youth, and families who seek assistance with a variety of mental health challenges, such as trauma, substance abuse, and emotional distress. Your expertise in family therapy or trauma-informed care will be particularly valuable in addressing the multifaceted needs of our patient population, each deserving quality care tailored to their individual circumstances.

Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field is required.

• Active and unrestricted licensure as a Professional Counselor (LPC) in the state of Texas.

• Minimum of 2 years of counseling experience, ideally in a clinical or community health setting.

• Proficiency with electronic medical records (EMR) and a strong technical aptitude to enhance patient management.
