Responsibilities
- Evaluate and treat patients to improve mobility, function, and strength; implement individualized rehabilitation programs
- Develop and modify treatment plans, monitor progress, and adjust interventions as needed
- Document evaluations, treatment notes, progress reports, and discharge summaries in the electronic medical record
- Supervise PT Assistants and Aides, coordinating care across therapy sessions
- Communicate with physicians and other members of the care team to ensure high-quality patient care
- Maintain HIPAA privacy standards; participate in privacy training and protect patient information
- Assist with scheduling, referrals, and follow-up activities; collaborate with front office staff as needed
- Demonstrate proficiency in DX and ICD-10 coding for proper billing of services
- Meet physical demands of the role, including standing/walking for long periods, lifting up to 50 pounds, and assisting with patient transfers

Requirements
- Doctor of Physical Therapy (DPT) degree
- Active Physical Therapist license in California or eligibility to obtain licensure with plan to test in the near term
- Relevant experience in physical therapy rehabilitation settings; home health or outpatient experience preferred
- Proficient with electronic medical records, documentation, and ICD-10 coding; strong adherence to privacy and compliance standards
- Ability to work effectively in an outpatient rehabilitation environment
- Continuing education courses encouraged to support ongoing professional development
