Work Environment

This position is situated onsite within a school environment in Kingston, NY, where you will work collaboratively with educators, administrative staff, and health professionals. Your daily responsibilities will involve direct patient care, health education initiatives, and the organization of community outreach programs aimed at overcoming barriers to health services. The role requires working from 9:00 a.m. to 6:00 p.m., with the flexibility to offer evening clinics until 8:00 p.m. on occasion, alongside participation in weekend community health events.

Key Responsibilities

• Conduct thorough health assessments and screenings for students and their families, identifying their unique healthcare needs.
• Diagnose and manage both acute and chronic medical conditions, focusing on preventive care and health education.
• Collaborate with schools and local organizations to develop and roll out effective health education programs tailored to community needs.
• Provide culturally sensitive care, advocating for the rights of patients and ensuring equitable access to healthcare services.
• Coordinate referrals and follow-up care to maintain continuity of care and ensure comprehensive treatment for patients.
• Document all patient encounters accurately in compliance with regulatory standards, utilizing electronic health records (EHR) systems effectively.
• Actively participate in community outreach initiatives and health fairs to promote health awareness and facilitate screenings.
• Stay informed about health trends, evidence-based practices, and policies pertinent to community health.

Patient Population / Client Focus

The primary focus of this role will be the school-aged population within Kingston and the surrounding areas. You will engage with diverse groups of students from various backgrounds, allowing for the assessment and management of both physical and mental health challenges. Your efforts will aim to equip them with the knowledge and tools needed to lead healthier lives, while also supporting their families.

Qualifications

• A Bachelor's Degree in Nursing from an accredited institution is required.
• An active and unrestricted Nurse Practitioner (APRN) license in New York State is essential.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by either the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP) is mandatory.
• DEA# and NPI# are required for prescribing medications.
• Strong clinical assessment, diagnostic skills, and an ability to work both independently and collaboratively in a school-based environment.
• Excellent communication and interpersonal skills are crucial for successful interactions with students, parents, and community stakeholders.

Preferred Experience

• At least 2 years of experience as a Nurse Practitioner, preferably in primary care, school, or family medicine settings.
• Experience in engaging with community health initiatives or public health programs is advantageous.
• Proficiency in the use of electronic health records (EHR) systems to manage and maintain patient data effectively.

Compensation and Benefits

The position offers a competitive salary ranging from USD 150,000 to 160,000 annually, dependent on experience and qualifications. Additionally, a sign-on bonus of up to $10,000 is available for newly hired practitioners.
