Key Responsibilities

• Conduct thorough health assessments, including history-taking and physical examinations, to establish accurate diagnoses and develop individualized treatment plans for residents.
• Order and interpret necessary diagnostic tests, such as blood work or imaging, and prescribe appropriate medications and therapies to manage acute and chronic conditions.
• Educate patients and their families on health maintenance, chronic disease management, and disease prevention strategies, tailoring information to individual patient needs.
• Work collaboratively with an interdisciplinary team to coordinate patient care, facilitate referrals to specialists, and ensure seamless transitions between care levels.
• Keep detailed and compliant medical records, ensuring all documentation is accurate and up-to-date, meeting both regulatory standards and best practices.
• Engage in ongoing professional development, including attending conferences and workshops to stay informed on the latest advancements in healthcare and best practices.

Patient Population / Client Focus

You will primarily serve residents within our Assisted Living Facility, predominantly older adults who may have multiple chronic health conditions. Understanding the unique needs of this population, you will leverage your clinical expertise to implement tailored treatment strategies that contribute to improved health outcomes and enhance their quality of life.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited institution.
• Current licensure and certification as a Nurse Practitioner.
• Specialization in an accredited Nurse Practitioner program is preferred.
• Strong clinical assessment skills and a demonstrated ability to diagnose and manage a wide variety of health conditions.

Compensation and Benefits
