Out-of-Home Care Case Manager

Function:

This position is responsible for the case management of up to 12-15 residential and foster youth. The OOHC Case Manager’s primary responsibility is to stabilize youth who are placed in out-of-home care by completing intakes, assessing for strengths and needs, developing and implementing a client-centered treatment plan, coordinating therapeutic, medical, and other services as necessary, completing visits with youth and their support system, and working collaboratively with the youth’s treatment team, support system, and designated placing agency worker.

Organizational duties & responsibilities:

1. The primary responsibility of all staff is to ensure the safety and well-being of all Ramey-Estep/Re-group (RE) clients.
2. Supports the mission, vision, and values of RE. Facilitates and adheres to the agency’s code of ethics, policies, and procedures.
3. Supports all functions that attain and maintain accreditation and compliance with regulatory agencies.
4. Supports and facilitates positive interaction with clients and staff by exhibiting both in-office and in-public when carrying out job duties: individual maturity, respect for others, a team-centered approach, maintenance of confidential information, and awareness and sensitivity to cultural and other differences in clients and staff.
5. Exhibits effective communication skills, including proper use of agency communication systems.
6. Participates in appropriate professional development programs to attain and maintain competency.
7. Effectively manages financial and physical resources to achieve the mission of RE.
8. Reports incidents of abuse or potential abuse involving clients to the appropriate authorities and RE.

Essential Duties and Responsibilities:

1. Ensures that each youth receives services appropriate to the youth’s individual treatment plan.
2. Enters Case Manager Documentation notes promptly, documenting all interactions with youth, family, involved agencies, schools, etc., concisely summarizing those interactions.
3. Completes required contracts with youth as outlined in contractual agreements.
4. Attends court as requested and advocates for the youth's best interest.
5. Develop a comprehensive treatment plan and conduct quarterly treatment plan reviews involving the youth’s family or other natural supports and any involved agencies.
6. Collaborate with the treatment team to develop a discharge and aftercare plan to ensure the permanency goal is achieved through scheduling visits, participating in treatment, etc.
7. Communicate regularly with each youth’s worker and family (as appropriate).
8. Assists the youth and their treatment team with meeting each youth's mental health needs by linking them with the community resources needed.
9. Attends all treatment team meetings and is prepared to offer suggestions concerning treatment strategies.
10. Assists in crisis intervention and de-escalation of youth.
11. Enhance professional growth by reading, attending conferences, and in-service training, including occasional presentations at in-service training.
12. Participates in an on-call schedule.
13. Ensures that State, placing agency, and Ramey-Estep documentation and all paperwork are completed promptly.
14. Adheres to the highest ethical standards to fulfill the mission of the Ramey-Estep Homes.
15. Serves as a role model, demonstrating appropriate skills and behavior to staff and youth, including hygiene, grooming, dress, language, and social skills.
16. Ensure all necessary records are maintained and kept confidential.
17. Ensures that appropriate discharge planning is in place for each youth.
18. Ensures aftercare requirements are completed with youth post-discharge from RESTEP.
19. Completes initial and ongoing Safe Crisis Management training as required.
20. Perform other duties as assigned.

Working conditions/environment:

1. Shifts may vary depending on staffing needs. RE is a 24-hour, 7-day-a-week, 365-day-a-year facility; therefore, varied shift hours will be required.
2. Holidays, weekends, and extra hours may be required.
3. Meals are provided for staff when on campus.
4. Intense, unpredictable population with the possibility of verbal and physical aggression.
5. A fast-paced environment with the need for quick decisions to deal with any crisis that may arise.
6. The environment is both indoors and outdoors. It can vary between the residential campus, foster homes, schools, communities, etc.

minimum job requirements:

Bachelor’s Degree in a human services field from an accredited school is required.

Prior experience in case management is preferred.

Must be at least 21 years of age.
Must have strong communication and organizational skills.
Some positions may require a valid driver’s license and be insurable.
Must have excellent communication and conflict-resolution skills.
Technical requirements include proficiency with Microsoft Word, Excel, PowerPoint, and any other applications the organization or regulatory agencies use.
Ability to understand and relate to the needs of clients from diverse backgrounds.
Ability to read, write, and converse in English.
Successful completion of a pre-employment drug screen.
Successful completion of a background screening.
Successful completion of a TB skin test or proof of a negative chest x-ray or other documentation.

Successful completion of Excellent Foundations
Maintain 40 hours of annual training (including 16 hours of SCM)
Maintain 16 hours of Safe Crisis Management training, meeting all physical requirements unless given an exemption by the CEO or designee
Maintain CPR/1 st Aid training
