Responsibilities

• Conduct utilization management reviews to determine medical necessity of services and optimize patient outcomes
• Analyze records and documentation to support appropriate levels of care and discharge planning
• Collaborate with physicians, nurses, social workers, and payers to coordinate care across transitions
• Monitor documentation, coding, and regulatory compliance for accuracy and completeness
• Participate in rounds and provide floor coverage as needed to support patient care and throughput
• Facilitate efficient care transitions, including discharge planning and post-discharge follow-up as assigned

Requirements

• Active RN license to practice in New Hampshire (or eligible)
• Nursing degree from an accredited program (ADN or BSN)
• Experience in utilization management, care coordination, or a related setting is preferred
• Ability to work effectively in a skilled nursing facility environment
• Strong communication and collaboration skills to work with interdisciplinary teams
