Work Environment

This position is primarily onsite, providing care directly in the homes of patients in the Palm Desert area. Working in home care allows for a more personal connection with patients, as you will be visiting them in their daily environment. You will utilize various tools and equipment typical in a home setting to facilitate therapy sessions. Your schedule will be Monday to Friday, from 9 AM to 5 PM, enabling a balanced work-life dynamic while maintaining the flexibility to adapt sessions to meet patients' needs.

Key Responsibilities

• Conduct thorough evaluations of patients to determine their physical therapy needs and develop individualized treatment plans tailored to their specific goals.
• Deliver one-on-one patient care focusing on orthopedic, post-surgical, and sports-related rehabilitation, along with strategies for neurologic recovery.
• Collaborate with referring physicians and a multidisciplinary care team to ensure a seamless transition for patients, facilitating communication and shared goals.
• Educate patients and their families on therapeutic techniques, exercise regimens, and recovery strategies that they can implement outside of therapy sessions.
• Document patient progress accurately and regularly, adjusting treatment plans as necessary to reflect ongoing assessment findings.
• Engage in professional development activities, maintaining current knowledge in physical therapy advances through internal continuing education opportunities.

Patient Population / Client Focus

As a Physical Therapist at Fortis Motion LLC, you will work with a diverse patient population, including elderly individuals recovering from surgeries, active adults with sports injuries, and patients with chronic conditions requiring rehabilitation. Your interventions will not only focus on restoring physical function but also on empowering patients to take an active role in their recovery, promoting self-efficacy and long-term health.

Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program.
• A valid California Physical Therapy License or eligibility to obtain one.
• Strong communication and interpersonal skills that facilitate patient engagement and team collaboration.
• A proactive and solutions-oriented mindset, coupled with a commitment to continued education and professional growth.

Preferred Experience

• At least 2 years of experience in a physical therapy setting, preferably in outpatient or home care environments.
• Previous experience with orthopedic and sports rehabilitation is advantageous.

Compensation and Benefits

We offer a competitive salary in the range of $100,000 to $120,000, commensurate with experience, along with a comprehensive benefits package that includes:
