Whether at home or in a facility, we hope to enhance the quality of life for your loved one.We are a dedicated team of local professionals focused on delivering personalized home health and hospice care with an emphasis on quality and excellence.**Bella Terra Home Health and Hospice of the Desert is growing and looking for experienced, compassionate Physical Therapist (PT) to join our team!** As a PT with Bella Terra, you will have the opportunity to advance your career while receiving strong compensation and excellent benefits.**BENEFITS*** Competitive compensation* Health, dental, vision, life, and disability insurance* 401(k) plan with generous company match* Critical illness benefit* Paid time off* Employee assistance program**Pay Range:** $55.00 - $70.00/hr**Job Summary**The Physical Therapist (PT) is responsible for providing physical therapy services and for adherence to all conditions outlined in the Personnel Services Agreement.**Responsibilities*** Provide physical therapy services to patients according to a written physician's plan of care* Be responsible for the ongoing interdisciplinary assessment and development of the individualized plan of care in partnership with the patient, representative, and caregiver* Provide counseling to patient, caregiver, and family* Provide education to patient and caregiver* Prepare clinical notes and progress summaries based on the attainment of goals* Communicate with the physician and other health care practitioners regarding the current home health plan of care* Initiate physical therapy program and instructs other personnel and/or family/caregiver in certain phases of physical therapy with which they may work with a patient* Instruct personnel and/or family/caregiver as to the goals of the physical therapy program for the patient by participating in case conferences* Train patient in the use of prosthetic device* Identify patient and family/caregiver needs for other home health services* Participate in discharge planning for patient* Provide in-service education programs for nursing organization personnel* Participate in peer consultation process* Supervise Physical Therapy Assistants**Requirements*** Active CA Physical Therapy License* Strong verbal and written communication skills* Strong organization skills* Possesses and maintains current CPR certification* Must be a licensed driver with an automobile that is insured in accordance with state or organization requirements and is in good working orderBella Terra Home Health and Hospice of the Desert **is part of the Cornerstone Group**with about 75 home health and hospice agencies throughout the country. **While we are part of a large family, we operate as a local team.**We understand we are nothing without great employees!
