Key Responsibilities\n

• Conduct thorough psychosocial assessments to understand the emotional and mental health needs of clients and their families.

• Utilize evidence-based therapeutic modalities, such as cognitive-behavioral therapy and solution-focused therapy, to provide individual, family, and group therapy sessions focused on coping with terminal illness and grief.

• Design and implement client-centered treatment plans that set measurable goals and interventions tailored to the unique needs of each client.

• Collaborate closely with a multidisciplinary team of healthcare professionals, including physicians, nurses, and spiritual care providers, to ensure a holistic approach to patient care.

• Facilitate crisis interventions and develop risk assessments and safety plans to help manage clients in critical situations.

• Maintain accurate documentation of client sessions in accordance with ethical, legal, and organizational standards.

• Lead support groups and psychoeducation classes to empower families and caregivers dealing with the complexities of hospice care.

Qualifications\n

• Possession of a Master’s degree in Social Work from an accredited university.

• An active state licensure as a Licensed Clinical Social Worker (LCSW) in California.

Compensation and Benefits\n
