Position: Health Coach / RN Case Management Location: Fully Remote Schedule: Monday–Friday Pay: $47/hr on W2 Contract: 3 Months CANDIDATE LOCATION REQUIREMENT: Candidates must reside in the Tri-State area: Pennsylvania (PA), New Jersey (NJ), or Delaware (DE). ⚠️ PA RN LICENSE IS COMPULSORY — candidates must have a current, active, unrestricted Pennsylvania RN license . What You’ll Do: • Provide telephonic health coaching and case management • Conduct clinical assessments and develop individualized care plans • Support members with chronic, acute, and complex conditions • Coordinate care with physicians, providers, and healthcare teams • Identify barriers to care and promote self-management • Provide patient education, advocacy, and healthcare navigation • Connect members with appropriate community and health plan resources What We’re Looking For: ✅ Active, unrestricted PA RN License – Required ✅ Must reside in PA, NJ, or DE ✅ Minimum 3 years of combined clinical, case management, disease management, or care coordination experience ✅ Strong clinical knowledge and communication skills ✅ Health coaching, case management, care coordination, or patient education experience preferred ✅ Comfortable working in a fully remote, telephonic environment Interested?
