Lead Case Manager, ODR - Housing & Aftercare Advocate

The People Concern

The People Concern seeks a Lead Case Manager for the LEAD ODR program in Santa Monica, CA. You will identify, engage, and support chronically homeless clients, coordinating with LAPD referrals and aftercare services to prevent entry into the jail system. In collaboration with the LEAD team and outside agencies, you will maintain client records, provide advocacy, and connect clients to housing and supportive services while upholding wellness and recovery standards. #J-18808-Ljbffr The People Concern
