Clinical Social Worker 2/3

Job Description The Clinical Social Worker intervenes with patients who have complex psychosocial needs, requires assistance with eligibility determination for social programs and funding sources, and facilitates community assistance from a variety of special funds and agencies. The CSW offers crisis intervention to pre‑ and post‑Kidney Transplant patients and families and coordinates and facilitates the development of a discharge plan of care for high‑risk patient populations. The Clinical Social Worker coordinates resources and services in response to a patient’s psychosocial needs while collaborating with Case Managers, Nursing Staff, Medical Staff and ancillary departments to support timely and safe discharge and to meet the continuing care needs within fiscal realities of the case. The CSW will practice within the scope of UCSF’s mission statement: Caring, Healing, Teaching, Discovering, and the values statement of UCSF’s Everyday PRIDE: Professionalism, Respect, Integrity, Diversity, Excellence. The CSW will participate in department education and practice improvement activities, works in a collaborative practice model, and facilitates care coordination with the multidisciplinary treatment team to optimize patient outcomes and enable timely discharge when the patient is medically ready. The CSW provides social work services to any patient regardless of gender, gender identity, sexual orientation, social, cultural, educational or religious background, language, age, ancestry, citizenship, color, national origin, race, presence of mental or physical disability, source of payment for care, medical condition, marital status or status as a veteran. The CSW III (LCSW) possesses advanced clinical knowledge and skills and participates in leadership activities including advanced program planning, provision of educational programs, clinical supervision, consultation to the clinical team and other departments, and the community. Department Description Dealing with an illness can be a very difficult time for patients and their loved ones. UCSF Health sensitive social workers in our Social Work department assist patients and their families cope with the many psychological and social problems that may arise when facing illness, hospitalization and medical treatment. Our social workers act as patient advocates by serving as counselors, helping to find resources and solve problems. Any concerns related to the psychological, practical and social well‑being of each patient or their loved ones may be discussed openly with our social workers. Responsibilities Provide psychosocial assessment and interventions based on preliminary risk screening; assess patient’s and family’s psychosocial risk factors through evaluation of prior functioning levels, support systems, reaction to illness and ability to cope for transplant patients on both an inpatient and outpatient basis. Intervene with patients and families regarding emotional, social and financial consequences of illness and/or disability; utilize support of CSW III as appropriate for families exhibiting complex dynamics that directly impact patient care and discharge. Serve as primary care manager for patients with significant psychosocial needs; work in collaboration with case managers to manage clinical resources and transition planning from admission through discharge; identify and eliminate barriers to delivery of required clinical services; facilitate timely discharge when medically ready. Meet with providers, care teams and patients to identify goals for medical readiness and formulate primary and backup discharge plans. Coordinate and lead multidisciplinary rounds; monitor discharge milestones; assign accountability for tasks and proactively identify and address or escalate barriers to timely discharge. Reassess patient as needed to make real‑time adjustments to discharge plan. Complete all required documentation in APeX screens and patient records. Participate in multidisciplinary team care, especially on complex psychosocial cases; attend and contribute to multidisciplinary rounds (MDRs). Participate in discharge planning for complex patients to drive timely discharge when the patient is medically ready and to provide appropriate linkage with post‑discharge care providers. Educate patients, families and physicians regarding post‑acute options and address issues of choice. Implement domestic violence, child/elder abuse and other reportable situation protocols in accordance with legal mandates and UCSF policy and procedure. Collaborate with Psychiatry and other teams on cases with psychiatric holds or primary mental health/substance abuse problems and on patients expressing suicidal or homicidal ideation. Apply knowledge and skills in substance use, mental illness, end‑of‑life bereavement, benefits and entitlements, and adjustment to illness. Serve as a resource person and provide counseling and intervention related to treatment decisions and end‑of‑life issues; facilitate completion of Advance Health Care Directive and the POLST; coordinate application for probate conservatorship when appropriate. Ensure timely and accurate documentation in the Medical Record and collect required statistical data. Attend department, hospital and community in‑services related to the field of practice. Participate in quality improvement activities and program planning for the Department, Hospital and Service Lines. Oversee and direct activities delegated to the Resource Coordinator and/or Patient Care Navigator. Under direction of the Social Work Supervisor, provide clinical supervision to CSW II staff for LCSW licensure in compliance with the California Board of Behavioral Sciences (BBS). Provide coverage as needed and as assigned. Advanced written and verbal communication skills required. Basic computer skills required. Collaborate and communicate with the multidisciplinary transplant team on evaluation, listing and management of pre‑ and post‑transplant patients. Develop materials and provide transplant education to patients, families, community and staff. Facilitate transplant support groups and transplant education seminars as appropriate. Minimum Requirements Masters degree from a CSWE accredited school of Social Work. Experience (internship/career) in a healthcare and/or hospital setting preferred. Knowledge and abilities essential to the successful performance of the duties assigned, including knowledge of relevant community resources, medical and psychological sequelae of illness and impact on social functioning. Demonstrates resourcefulness, effective written and oral communication, diplomacy, and organizational and analytic skills. Self‑directed, assertive and creative in problem solving, systems planning and patient care management in a high‑volume work environment. Ability to work effectively and collaboratively with interdisciplinary teams. Basic pager, telephone and computer skills including the use of the internet. Flexibility to orient and work at all UCSF Medical Center locations. Licensed in Clinical Social Work in the state of California / Board of Behavioral Sciences (for CSW III). Three years of experience in a healthcare setting required and hospital setting preferred (for CSW III). Preferred Qualifications Current California State License of Clinical Social Work in good standing. Three years of experience in a healthcare setting required and hospital setting preferred. Licenses and Certifications Licensed in Clinical Social Work in the state of California / Board of Behavioral Sciences.
