Work Environment

This position is offered on an onsite basis, ensuring close collaboration with interdisciplinary teams, including physicians, nurses, and support staff within our hospital environment. Your workweek will consist of approximately 40 hours, typically from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., as well as participation in weekend health fairs and community outreach events designed to improve health literacy and access.

Key Responsibilities

• Conduct comprehensive health assessments and screenings, identifying health risks and formulating appropriate care plans for individuals and families.
• Diagnose and manage both acute and chronic medical conditions, ensuring a holistic approach that incorporates preventive care and health promotion measures.
• Collaborate closely with community organizations and stakeholders to design, implement, and evaluate health education programs aimed at improving health knowledge and behaviors.
• Provide culturally sensitive care, advocating for patients’ rights, healthcare access, and facilitating communication between patients and the healthcare system.
• Coordinate patient referrals and ensure follow-up care to maintain the continuum of care, contributing to improved patient outcomes.
• Maintain accurate and detailed medical records, ensuring compliance with local, state, and federal regulatory standards.
• Participate actively in community outreach events and health fairs to deliver health screenings and educational sessions, enhancing community health awareness.
• Stay current with emerging health trends, evidence-based medical practices, and relevant policy changes to optimize patient care delivery.

Qualifications

• Current certification as a Nurse Practitioner is required.
• An active nursing license in the state of New York is essential.

Preferred Experience

Compensation and Benefits

• We offer a competitive salary range of USD 180,000 – 200,000 per year, commensurate with experience and qualifications.
