Job Title: Registered Nurse (RN) Case Manager – Home Health | 1:1 Care, Autonomy, Flexible Field Role

Location: Greater Twin Cities Metro Area (21-county service area)

Schedule: Full-Time, Monday to Friday, 8:00 AM – 5:00 PM, 1 weekend per month, 1 on-call shift per week

Compensation: $85,000 – $130,000 annually plus $6,000 in first-year bonuses

What You’ll Do

• Manage a home health caseload across homes and community settings.

• Complete OASIS assessments and individualized care plans .

• Provide skilled nursing care, education, and care coordination .

• Work closely with physicians, therapists, and interdisciplinary teams.

• Support patients in improving function, safety, and independence .

• Maintain accurate, timely documentation .

What We’re Looking For

• Active Minnesota Registered Nurse (RN) license.

• 3+ years of RN experience, home health preferred.

• Strong clinical judgment, assessment, and documentation skills .

• Ability to work independently in a field-based role .

• Reliable transportation.
