Job Overview: As a Nurse Practitioner in the Home Health unit, you will play a vital role in providing comprehensive healthcare services to patients in their homes. This fee-for-service position involves serving as a NY Medicaid Provider and requires ANCC certification. Key Responsibilities:

• Conduct patient assessments, diagnose medical conditions, and develop appropriate care plans.
• Collaborate with a multidisciplinary team to ensure comprehensive and coordinated patient care.
• Provide education and support to patients and their families regarding health management and preventive care.
• Administer prescribed medications and treatments.
• Maintain accurate and up-to-date patient records.

Qualifications:

• Nurse Practitioner (NP) degree
• ANCC Certification
• Valid NY Medicaid Provider status
• Previous experience in home health preferred
• Strong communication and interpersonal skills

Working Conditions:

• Per Diem position with flexible scheduling
• Day shifts
