Home Health Aide – Skilled Home Care

Schedule

Day/shift hours can vary based on the needs of the Home Care office: Mon - Fri 8a to 5p, Mon - Fri 9a to 6p, Thurs - Mon or Fri to Tues shifts, and alternating weekend shifts.

Assignment Details

Home skilled visits – 6-8 per day.

Requirements

• 1 year of skilled home care experience (private duty nursing experience not accepted).
• Experience with OASIS training and home health documentation.
• IV skills and wound care experience.
• Familiarity with general medical diagnoses as well as high-risk diagnoses including heart failure, COPD, myocardial infarction, pneumonia, and stroke.
• Post-operative care experience.
• Current BCLS certification.
• Valid driver's license and car for home visits.
• COVID vaccination or approved exemption, and flu vaccination or declination are required, with mandatory mask wearing at all times while at work.

Responsibilities

• Conduct cardiopulmonary assessments and basic wound care.
• Manage ABI compression dressing and wound vac dressings.
• Perform catheter care and changes, staple/suture removals, and lab draws including venipuncture.
• Provide SQ/IM injections and conduct PICC line or Midline care and dressing changes.
• Utilize glucometers and pro-time machines for patient assessments.
• Complete documentation using BayCare Forms, BayCare Scheduling, and Ace Timesheet systems.
