This role will be primarily remote, allowing for a flexible work-life balance while still providing exceptional care to clients. You will be part of a multidisciplinary team focused on holistic healing approaches, collaborating with healthcare professionals remotely as well as engaging in periodic case conferences. The remote setup enables ease of access for clients, whether through virtual meetings or phone calls.

Key Responsibilities\n

• Plan, coordinate, and implement individualized support packages tailored to clients’ unique circumstances, aiming to facilitate their recovery journeys.

• Conduct thorough interviews to assess clients’ psychological and emotional conditions, determining their strengths, weaknesses, and immediate needs.

• Develop and establish personalized measurable goals for each case, ensuring a clear path to recovery and well-being.
\n
• Continuously monitor and evaluate clients’ progress, adjusting treatment plans as necessary to fit their evolving situations effectively.

• Provide informed counseling and guidance about the best courses of action during sessions, empowering clients to make informed decisions.

• Maintain meticulous records and documentation of patient status, progress, and treatment plans in compliance with ethical and legal requirements.

• Participate in case conferences and, when required, provide testimony in court to advocate for client needs.
\n
• Act as the key-worker in the treatment process while collaborating closely with other professionals to ensure holistic patient support.

• Facilitate referrals to community services and additional resources that contribute to clients’ recovery and personal growth.

Patient Population / Client Focus\n

Your primary focus will be working with clients facing challenges such as neglect, child abuse, domestic violence, mental health issues, and parental substance abuse. This role requires you to approach each case with empathy and a strong understanding of the complex factors that influence these issues. Qualifications\n

• A Master's Degree in Social Work (MSW) from an accredited institution.
