Work Environment

As a Family Nurse Practitioner at our hospital, you will immerse yourself in an environment that emphasizes teamwork and patient-centered care. Our facility is equipped with advanced medical technology, and our staff is a diverse group of professionals who are passionate about making a difference. You will work primarily from 9:00 a.m. to 6:00 p.m., with some evening clinics extending to 8:00 p.m. Participation in weekend community health events may also be required, further underscoring our commitment to outreach and education.

Key Responsibilities

• Conduct thorough health assessments and screenings for a diverse range of individuals and families, applying your clinical expertise to identify health needs accurately.
• Utilize evidence-based practices to diagnose and manage both acute and chronic medical conditions, working closely with patients to develop personalized care plans.
• Collaborate with community stakeholders and organizations to design and implement health education programs aimed at preventive care and disease management.
• Provide culturally sensitive care, advocating vigorously for the rights and needs of patients, ensuring they have equitable access to healthcare resources.
• Coordinate referrals and follow-up care, ensuring seamless continuity of care and positive patient outcomes.
• Maintain comprehensive medical records in compliance with regulatory standards, leveraging electronic health record (EHR) systems for improved patient management.
• Lead and participate in community outreach events and health fairs, offering screenings and health education to raise awareness about public health issues.
• Stay abreast of local and national health trends, policies, and best practices to ensure that your care strategies remain effective and current.

Qualifications

• Current certification as a Nurse Practitioner.
• Active state nursing license in Florida.
• A Master's degree in Nursing from an accredited educational institution.

• Commitment to addressing social determinants of health and advocating for health justice, particularly within vulnerable communities.

Preferred Experience

• A minimum of 3-5 years of clinical experience, especially in community health or public health settings.

• Previous involvement in community health initiatives or programs that address public health issues.

Compensation and Benefits
