Key Responsibilities

• Conduct thorough patient assessments and detailed medical histories to enable accurate diagnoses and personalized treatment plans.
• Collaborate with physicians, nurses, and other healthcare professionals to ensure comprehensive care for patients throughout different stages of life.
• Perform physical examinations, order and interpret diagnostic tests, and create tailored treatment plans that promote optimal health outcomes.
• Administer vaccinations and medications; educate patients about preventive health measures and necessary lifestyle modifications.
• Monitor and manage chronic conditions, coordinating referrals to specialty care when necessary to ensure patient needs are met effectively.
• Diligently document patient interactions and care provided in the EPIC EMR system to maintain accurate patient records.
• Engage in quality improvement initiatives, continuously honing your skills and staying updated with the latest medical advancements in family medicine.
• Stress the importance of preventive care to enhance patient wellness and improve overall quality of life.
• Foster strong, supportive relationships with patients and their families, delivering compassionate and patient-centered care.

Patient Population / Client Focus

Qualifications
