Key Responsibilities

• Conduct thorough assessments of adult patients, including gathering medical histories, performing physical examinations, and ordering necessary diagnostic tests to understand each patient’s unique health profile.
• Diagnose chronic and acute illnesses, developing comprehensive treatment plans in consultation with interdisciplinary teams of physicians and specialists.
• Utilize evidence-based practices to design and implement personalized treatment plans that cater to individual patient preferences while promoting optimal health outcomes.
• Prescribe medications thoughtfully and manage treatments, regularly reviewing and adjusting plans based on patient responses and emerging health information.
• Educate patients and their families about disease management, prevention strategies, and healthy lifestyle choices that empower them to take control of their health.
• Perform medical procedures within the scope of your practice and ensure that patient safety and comfort remain top priorities.
• Maintain meticulous electronic medical records, documenting all patient interactions and treatment plans accurately to facilitate seamless communication among the care team.
• Engage actively in team meetings and care planning discussions to coordinate multidisciplinary interventions and ensure continuity of care across our healthcare services.
• Stay abreast of advancements in adult health care practices to continually enhance knowledge, skills, and patient care strategies.

Patient Population / Client Focus

You will primarily be working with adult patients residing in our Assisted Living Facility who may have both chronic conditions and acute health issues. Many residents have complex medical histories requiring compassionate, informed approaches to care. By fostering strong relationships and addressing their physical and emotional needs, you will play a vital role in enhancing their overall quality of life.

Qualifications

• Must hold a current certification as a Nurse Practitioner.
• Active nursing license in the state of Texas is required.
• A Master’s degree in Nursing from an accredited program is mandatory.
• At least 2-3 years of practical experience in adult primary care settings, preferably in assisted living or similar environments.
• Demonstrated ability to utilize clinical judgment and critical thinking skills to provide evidence-based care.
• Exemplary communication and interpersonal skills, essential for establishing rapport with patients and teamwork with colleagues.

Preferred Experience

• Experience working in assisted living facilities or geriatric care settings is highly desirable.
