Belares’ Family Care Center is actively seeking an experienced and compassionate Family Nurse Practitioner to become a vital member of our healthcare team in Laredo, Texas. In this full-time role, you will provide high-quality, patient-centered care within a unique concierge medicine model that emphasizes accessibility and personal engagement. Your responsibilities will encompass direct patient care, including physical assessments, diagnostic test interpretation, chronic condition management, and care coordination.

Work Environment

This role will operate onsite at our facility, engaging directly with patients in a supportive and collaborative environment. Our setting is designed to remove barriers to care, providing convenience and accessibility to our patients. The work schedule typically involves weekdays from 8:00 a.m. to 5:00 p.m., with some evening clinics until 8:00 p.m. and occasional weekend events. This structure allows our practitioners to maintain a healthy work-life balance while contributing meaningfully to patient care.

Key Responsibilities

As a Family Nurse Practitioner at Belares’ Family Care Center, your primary responsibilities will include:

• Conducting comprehensive health assessments and screenings for individuals and families, addressing both acute and chronic medical issues.
• Diagnosing and managing a range of health conditions while implementing preventive care measures and health promotion strategies tailored to the diverse needs of our patient population.
• Collaborating with local community organizations to design and execute health education programs that promote wellness and disease prevention.
• Providing culturally competent care, ensuring that each patient's Rights and access to healthcare services is advocated for and respected.
• Coordinating referrals and follow-up care, facilitating seamless transitions between various healthcare services to maintain continuity of care.
• Maintaining impeccable and reliable medical records in compliance with relevant regulations, enhancing effective patient communication and care planning.
• Actively participating in community outreach programs and health fairs, contributing to public health initiatives and screenings that enhance community health literacy.
• Staying informed of current community health trends and evidence-based practices, integrating this knowledge into patient care to achieve optimal health outcomes.

Patient Population / Client Focus

Our Family Nurse Practitioner will focus on a diverse patient population, predominantly consisting of families and individuals from Laredo and the surrounding areas. Qualifications

To be considered for this role, candidates must meet the following qualifications:

• Active Nurse Practitioner certification and a valid state license to practice in Texas.

Preferred Experience

Compensation and Benefits
