Work Environment

Our ideal candidate will work in an onsite setting, engaging directly with patients in their homes and at our medical center. You will have a flexible work schedule, typically from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Additionally, you will participate in community outreach events and health fairs, which may take place on weekends. The position offers approximately 40 hours of work per week and supports a collaborative, team-oriented environment that emphasizes compassionate care.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, identifying their physical, emotional, and social health needs.
• Diagnose and manage a variety of acute and chronic medical conditions, providing preventive care and health promotion initiatives tailored to patient demographics.
• Collaborate with local agencies and stakeholders to develop and implement community health education programs, addressing current health trends and concerns.

Compensation and Benefits
