Key Responsibilities\n

• Assist primary care providers in identifying, treating, and managing mental health and psychosocial issues.

• Conduct comprehensive patient intakes focusing on thorough diagnostic and functional assessments, followed by tailored treatment recommendations for each patient.

• Provide brief, evidence-based interventions aimed at addressing and improving patients' mental health needs.
\n
• Coordinate with primary care providers to identify clients who may benefit from specialized mental health services and facilitate appropriate referrals.

• Implement follow-up strategies to monitor patient progress and adjust treatment plans as necessary to optimize outcomes.

• Develop and execute relapse prevention plans for patients, supporting their stability and ongoing growth.
\n
• Identify at-risk patients and devise proactive plans to avert deterioration of their conditions.
\n
• Document patient progress thoroughly in their health records while keeping primary care providers informed of treatment plans and progress updates.

• Educate patients on self-management techniques and coping strategies, empowering them to actively engage in their recovery.

• Facilitate communication within the interdisciplinary team to evaluate outcomes and improve program efficacy continuously.

• Maintain adherence to the Code of Ethical Conduct for Clinical Professionals, ensuring a high quality of care.

Qualifications\n

Compensation and Benefits\n
