Your Health is seeking a Registered Nurse to serve as Visiting Nurse across the Hilton Head Island area. The Nurse Case Manager will travel to patients' homes and facilities, coordinate care with providers, and initiate telemedicine visits to ensure timely, quality care. Responsibilities include reviewing medical records, enrolling patients in Care Management, developing plans of care, and communicating with the care team to optimize outcomes. #J-18808-Ljbffr Your Health
