Key Responsibilities\n

• Conduct thorough psychosocial assessments and develop individualized treatment plans tailored to meet each client's unique needs.

• Diagnose and treat a range of mental health disorders, adjusting care plans in response to changes in patient status.

• Provide continuous support and counseling in both individual and family therapy sessions, facilitating a safe space for clients to explore their thoughts and feelings.

• Collaborate with a multidisciplinary team to create effective discharge plans that ensure continuity of care, coordinating services and referrals as necessary.

• Educate and guide patients on appropriate community resources, treatment programs, support groups, and additional mental health providers to enhance their support systems.

• Actively participate in team meetings to discuss patient needs and care strategies, contributing to the overall improvement of the counseling program.

• Address crises effectively by assessing urgent situations and connecting patients with the necessary resources, ensuring a timely and appropriate response.

• Maintain compliance with federal, state, and local regulations, as well as internal clinical standards, to uphold the highest quality of care.

• Serve as an advocate for clients and their families, facilitating effective communication with multidisciplinary teams and external agencies.

• Identify and manage patients who may be at risk for delayed discharge, readmission, or require complex care coordination.

Compensation and Benefits\n

The annual salary for this position ranges from USD 90,000 to 100,000, commensurate with experience. Additionally, we offer a sign-on bonus of up to $1,000 as an incentive for qualified candidates. Other benefits include:
