LPN - Skilled Nursing

This is a 37.5-hour per week position, with a shift from 3:00 PM to 11:30 PM. Every other weekend and holiday availability is required. This position may require floating to other units and possible duty as a CNA based on staffing needs. Job Description The primary functions of an LPN include assisting with the assessment, planning, implementation, and evaluation of the care provided to all patients during their shift. LPNs are responsible for meeting established standards of skilled nursing practice and collaborating with the care team to ensure Person-Centered care. Responsibilities include administering medications and treatments under the supervision of a Registered Nurse, taking and reporting vital signs, maintaining clinical competence, and accurately documenting in EMR/Point Click Care. Responsibilities Assist in the assessment, planning, implementation, and evaluation of patient care. Administer medications and treatments under RN supervision. Monitor and report changes in patient conditions to the nursing team. Provide ethical and professional care within the defined scope. Coordinate care with the interdisciplinary team. Document patient care accurately and thoroughly in EMR/Point Click Care. Required Experience / Certifications / Licensure Valid and active LPN license. Experience with Point Click Care is preferred. Current health assessments per facility policy, including MMR, Hepatitis B, and Tdap immunizations. Negative TB test and fit test completion. Assignment Duration This is a 13-week contract position.
