What You'll Do

• Evaluate patients' physical therapy needs and develop individualized care plans
• Implement treatment strategies to improve patient mobility and independence
• Communicate effectively with patients, families, and healthcare team members
• Document patient care activities accurately and promptly in electronic medical records
• Assisting with patient transfers and positioning as needed
• Monitor progress and modify treatment plans to meet patient goals
• Educate patients and caregivers on therapeutic exercises and safety

What You Need

• Valid IL state license in Physical Therapy
• CPR certification
• Experience in outpatient or home health setting preferred

• Strong communication skills
• Ability to work independently and as part of a multidisciplinary team
• Compassionate approach to patient care
