Speech Therapist, Home Visits

Job Overview Join our caring community as a part‑time Speech Language Pathologist at CenterWell Home Health. You will work 20‑24 hours per week, provide personalized care in patients’ homes, and help them regain strength, mobility, and independence. Responsibilities Evaluate, direct, and provide speech/language pathology services to patients in the home or facility. Develop and review Plans of Treatment (PoT) and Plans of Care (PoC). Assess and treat disorders of speech, voice, language, hearing, and swallowing. Administer and interpret diagnostic tests, including auditory logic screening. Observe, record, and report patient condition changes and treatment responses to supervisors or physicians. Provide instruction and training to patients on alternative communication systems when appropriate. Offer counseling and instruction to patients, families, and healthcare staff. Maintain required documentation and submit written reports on evaluations, visits, care plans, coordination activities, and progress. Participate in care coordination activities and discharge planning. Adhere to professional conduct standards regarding confidential information. Required Qualifications Minimum six months of experience as a speech therapist or speech-language pathologist. Current, unrestricted state license. Current CPR certification. Valid driver’s license, auto insurance, and reliable transportation. Preferred Qualifications Home Health experience. Pay Range & Hours $54.00 – $76.00 per visit/unit. Base pay: $85,400 – $117,500 per year (for full‑time equivalent). Scheduled weekly hours: 20–24 hours. Additional Information Tuberculosis screening required as part of Humana’s TB program. Driver safety program requires valid state driver’s license and liability insurance meeting specified limits.
