The nurse develops nursing plans of care with patients and their designated care partners, ensuring alignment with cultural beliefs and values. Responsibilities Interact with co-workers, patients, families/designated care partners, and other staff in accordance with Jefferson values. Integrate evidence-based practice, clinical expertise, and patient/care partner preferences to deliver optimal healthcare. Partner with patients and their families/designated care partners to identify learning needs and provide education that is clear, understandable, and affirming. Minimize risk of harm by following established nursing practices, protocols, and individual performance standards. Use information technology to communicate, manage knowledge, mitigate error, and support ethical decision-making. Utilize data to monitor care outcomes and apply improvement methods to design and test changes, continuously advancing quality and safety. Function effectively within nursing and inter‑professional teams, fostering open communication, mutual respect, and shared decision‑making. Recognize patients, families/designated care partners in providing safe, compassionate, coordinated care respecting personal preferences, priorities, needs, and cultural values. Create an environment of belonging and inclusion, demonstrating problem‑solving and conflict‑resolution skills. Demonstrate commitment to ongoing learning, professional growth, and serve as a primary preceptor for appropriate staff members. Actively participate in hospital and practice setting based committees, including safety and quality improvement. Show willingness to give and receive feedback, integrating appropriate changes into practice. Qualifications Specialized Diploma or accredited Bachelor of Science in Nursing graduate required. Knowledge and experience in patient‑and‑family‑centered practice, cultural awareness, effective communication, partnership with patients/families, delegation, and patient assessment. Proficiency in patient and care partner education, medication administration, discharge planning, health information documentation, and utilization of various assessment tools and nursing diagnosis. BLS certification (Basic Life Support). Active RN license in Pennsylvania required. Preferred Qualifications Bachelor’s Degree (BSN) or completion of BSN or appropriate advanced degree within three years of hire. One year of previous RN experience in an acute‑care setting. Additional Qualifications Active RN license in the state of practice. Physical Demands Lifts and carries 40–50 lbs. May push/pull patients on bed or stretcher, perform lateral transfers of up to 50 lbs. of the patient’s weight, stand/walk continuously, and perform CPR or other required activities. Patient transporters may walk 8–10 miles per shift.
