Therapy/Rehabilitation – PT Home Health

Schedule

Shift: 8-hour days (08:00 - 16:30). This facility cannot accommodate block scheduling. No weekends required.

Assignment Details

May visit up to 6 patients per day; 30 productivity points are the standard. Guaranteed hours: the facility can call off up to one shift per pay period.

Requirements

• 1 year of skilled home health experience required.
• BCLS certification required.
• Pending license accepted.
• Oasis documentation experience required.
• Transportation required for client visits.

Responsibilities

• Deliver high-quality care to home health patients.
• Assess patient needs and develop individualized treatment plans.
• Collaborate with other healthcare professionals to optimize patient outcomes.
• Document patient progress and update records accordingly.
• Provide education and support to patients and their families.
