Key Responsibilities\n

• Conduct thorough assessments to ascertain the mental health needs of clients, creating individualized treatment plans based on these needs.

• Maintain open lines of communication with supervisors, direct care team members, families, and other stakeholders regarding client progress and adjustments to therapy plans.

• Develop and propose a tentative schedule for appointments, which may include therapy sessions and follow-up home visits.

• Coordinate home visits as necessary to execute program plans effectively, ensuring that the client’s environment supports their treatment goals.

• Ensure the proper implementation of customized program plans, facilitating effective therapy interventions for clients.

• Actively participate in Interdisciplinary Team Meetings and other quality-focused discussions to enhance patient care.

• Create and maintain comprehensive monthly and quarterly progress notes to track client advancements and therapeutic outcomes.

• Organize training and development initiatives to ensure that all staff members are proficient in the program's implementation and documentation processes.

• Provide ongoing follow-up through interviews and observations, supporting clients' continual progress in their mental health journey.

• Serve as a liaison between clients, families, and the mental health community to promote effective development and support strategies.

• Contribute to the creation of training materials designed to enhance program development and service delivery.
