About the Role:

The Community Health Representative Coordinator plays a pivotal role in enhancing public health outcomes by leading and supporting a team of community health representatives. This position is responsible for coordinating outreach programs, facilitating health education, and ensuring effective communication between healthcare providers and the community. The coordinator will develop strategies to address health disparities and promote wellness initiatives tailored to diverse populations. By managing resources and partnerships, the role ensures that community health representatives are well-equipped to deliver culturally competent services. Ultimately, this position aims to improve access to healthcare, increase community engagement, and foster healthier communities across the United States.

Minimum Qualifications:

• Bachelor’s degree in Public Health, Community Health, Nursing, Social Work, or a related field. An Associate of Applied Science in Human Services or an equivalent combination of education, training, and relevant work experience may be substituted.
• Experience working in community health or public health settings preferred.
• Proven experience in program coordination or management, preferably in a health-related context.
• Strong understanding of community health principles and cultural competency.
• Excellent communication and interpersonal skills
• Knowledge of growth and development principles across the lifespan, including pediatric, adult, and geriatric populations, with the ability to assess needs, interpret information, and support age-appropriate health education and services.
• Ability to work independently in a village-based setting and manage administrative responsibilities, including correspondence, documentation, recordkeeping, and reporting requirements
• Demonstrated organizational, leadership, and problem-solving skills, with the ability to coordinate activities, supervise staff, and manage multiple priorities effectively.

Preferred Qualifications:

• Master’s degree in Public Health or a related discipline.
• Experience working with diverse populations, including underserved or marginalized communities.
• Familiarity with data collection, analysis, and reporting tools used in public health.
• Certification as a Community Health Worker or similar credential.
• Bilingual abilities, particularly in Spanish or other languages commonly spoken in the community.

Responsibilities:

• Supervise and support community health representatives in their daily activities and outreach efforts.
• Develop, implement, and evaluate community health programs that address local health needs and priorities.
• Coordinate training sessions and professional development opportunities for community health representatives.
• Collaborate with healthcare providers, public health agencies, and community organizations to enhance service delivery.
• Monitor program outcomes and prepare detailed reports to inform stakeholders and guide continuous improvement.
• Manage budgets, resources, and schedules to ensure efficient program operations.
• Serve as a liaison between the community, healthcare systems, and public health entities to facilitate communication and trust.
