Description

• Conduct thorough evaluations in both inpatient and outpatient settings to craft personalized treatment plans
• Implement physical therapy strategies aimed at improving mobility, strength, balance, and overall function
• Engage collaboratively with a multidisciplinary team including nurses, physicians, and other rehabilitation specialists
• Educate patients and their families regarding therapeutic goals, discharge plans, and home exercise regimens
• Maintain comprehensive documentation of evaluations, treatments, and patient progress as per facility standards
• Actively participate in care team rounds and interdisciplinary meetings to facilitate cohesive care

Education

• Doctorate in Physical Therapy (DPT) or an equivalent accredited degree

Certifications

• State license as a Physical Therapist
• CPR certification preferred
• Additional certifications in specialized areas (such as neurological or geriatric care) beneficial but not mandatory

Skills

• Prior experience preferred but not necessary
• Able to handle diverse patient populations with varying medical and mobility challenges
• Exceptional communication and teamwork abilities
