Client/Position Details HCN is seeking a Registered Nurse (RN highly preferred) or Licensed Practical Nurse (LPN w/ several years of experience) to provide Home Health Private Duty Nursing (PDN) services in and around Lancaster, OH (Fairfield County) for a loving young adult male patient with an amazing family (active parental caregivers + in-home care team). Specialized PDN Requirements Minimum of 1 year experience with vent care and tube feeding / G-tube care (2+ years preferred) Minimum of 2 years of experience in paid home health or visiting nurse setting (home health agency experience a plus) Strong medication administration skills Core Duties Ventilator & Tracheostomy Management G-Tube & Enteral Feeding Care Medication Administration Follow physician orders and established Plans of Care Excellent clinical documentation Communicate patient changes to the RN Case Manager + Care Team members Qualifications Active Ohio RN or LPN license Minimum of 1 year experience with vent care and tube feedings / G-tube care expertise (2+ years preferred) Minimum of 2 years of experience in paid home health or visiting nurse setting (home health agency experience a plus) Strong medication administration skills Reliable transportation and punctuality Active CPR and First Aid certificates highly desired; agency will provide renewal/recertification if required Good interpersonal communication skills - written and verbal Desirable traits: reliable, honest, professional, compassionate, detail-oriented (in care + documentation), accountable, friendly and accommodating, respectful Ability and willingness to perform all required duties including essential physical demands (listed below) Current driver\'s license or state ID, plus reliable transportation, are required Excellent attendance record with past employers Why HCN? Position Summary A successful PDN at Home Care Network, Inc., performs skilled nursing functions in accordance with the laws and guidelines of the agency and professional standards. The LPN/RN delivers care to clients as delegated by the RN Case Manager. Reports to RN Case Manager; Clinical Administrator/Director of Nursing Essential Functions/Areas of Accountability Provides professional nursing care under the direction of a Registered Nurse; communicates status/condition changes to the RN per agency guidelines Provides skilled nursing visits as directed by the RN Case Manager; follows the care plan established by the RN Recognizes and reports changes in client condition to the Case Manager and physician Communicates information in a timely and complete manner; documents in the clinical record Scope includes personal care, assessment, medication monitoring/administration, client teaching, emergency intervention, and nursing services per scope of practice Licensed personnel may perform venipuncture for obtaining blood samples where applicable Promotes safety and a safe environment for clients and coworkers Adheres to safety/infection control practices and agency policies Recognizes and responds to unsafe situations; demonstrates safe practice with equipment Suggests safety improvements; adheres to agency policies and professional standards Maintains confidentiality; participates in Quality Improvement activities Performs other related duties as needed Essential Physical Demands Frequent standing, walking/bending/stooping; occasional sitting Weight handling: 10-20 lbs frequently, 20-30 lbs infrequently; carry/lift 5-10 lbs occasionally, 10-30 lbs infrequently #J-18808-Ljbffr Home Care Network, Inc
