Position Summary

The Nurse Supervisor oversees the delivery of resident care during assigned shifts, ensuring residents receive safe, timely, and person-centered care. This position assesses changes in resident condition, provides clinical direction to caregivers, maintains accurate documentation, and serves as a key resource for residents, families, and the care team.

The Nurse Supervisor also supports staffing, caregiver performance, training, and shift operations while working closely with the Director of Resident Care Services, Wellness Director, and Executive Director.

What You’ll Do

Clinical & Resident Care

• Assess residents and identify medical concerns or changes in condition; ensure appropriate documentation and follow-up.
• Notify physicians and family members of significant changes in condition or medical concerns.
• Review physician orders, laboratory requests, and follow up on resident needs.
• Perform finger sticks and insulin injections for diabetic residents as directed.
• Monitor special dietary requirements and ensure care staff follow resident care orders.
• Provide clear direction to caregivers regarding resident care and daily needs.
• Coordinate medical appointments and follow up on resident care needs.
• Proactively coordinate appropriate medical care to help address concerns and reduce unnecessary emergency room visits and 911 calls.
• Work with Home Health and Hospice providers to support residents and ensure care plans are followed.
• Respond to resident and family questions and concerns in a professional and timely manner.

Team Leadership & Supervision

• Assign caregiver duties and monitor completion of assigned responsibilities.
• Monitor caregiver performance and adherence to policies, procedures, and quality care standards.
• Provide feedback regarding caregiver performance to leadership and contribute to performance evaluations as requested.
• Manage shift coverage when staffing needs or call-outs occur.
• Participate in caregiver orientation, training, and ongoing education.
• Provide hands-on leadership and promote a positive, collaborative work environment.

Operations & Compliance

• Maintain appropriate levels of medical and care supplies, including stocking and ordering as needed.
• Ensure equipment is used properly and provide instruction to team members when necessary.
• Maintain resident confidentiality and protect sensitive information.
• Promote teamwork, professionalism, and a resident-centered culture.
• Perform other duties as assigned.

Requirements:

Qualifications

• Current, active LVN license in the applicable state.
• Previous experience caring for older adults in a healthcare or senior living setting.
• Previous supervisory or leadership experience preferred.
• Strong clinical documentation and communication skills.
• Ability to build effective, professional relationships with residents, families, team members, and healthcare partners.
• Ability to provide clear direction and support to caregivers.
• Strong organizational skills with the ability to prioritize and respond to changing resident needs.
• Professional, compassionate, and resident-focused approach.
• Ability to represent the community in a positive and professional manner.

PHYSICAL REQUIREMENTS / WORKING CONDITIONS:

• Ability to frequently talk and listen in order to communicate effectively with residents, families, visitors, and team members.
• Ability to climb, reach, bend, stoop, twist, and move throughout the community as needed.
• Medium work: Ability to exert 20–50 pounds of force occasionally, 10–20 pounds frequently, and/or up to 10 pounds constantly to lift, carry, push, pull, or otherwise move objects.
• Ability to see sufficiently to read and write, administer or assist with medications as directed, and visually interact with residents, families, and team members.
• Ability to hear and speak sufficiently to communicate effectively with residents, families, visitors, and team members.
• Ability to safely move throughout the community and perform duties in a fast-paced senior living environment.
