The center is equipped with the latest physiotherapy tools and technologies designed to assist in delivering exceptional patient care. As a part of a collaborative and multidisciplinary team, you will work closely with specialists, including occupational therapists, physicians, and nursing staff, to create comprehensive rehabilitation plans for our clients.Key ResponsibilitiesThe Physical Therapist will be responsible for the following:Developing and implementing personalized treatment plans that focus on improving muscle function and reducing pain for patients with orthopedic conditions.Conducting thorough evaluations through diagnostic assessments of muscle, nerve, joint, and functional abilities to determine the best course of treatment.Supervising and guiding Physical Therapy Assistants to ensure effective delivery of care according to established protocols.Administering manual therapy techniques, therapeutic exercises, and other modalities, including traction and massage, to enhance patient recovery.Engaging in the ongoing assessment of client progress and adjusting treatment plans as necessary to meet evolving needs.Documenting clinical findings, care plans, and discharge summaries in compliance with regulatory standards and organizational policies.Participating in interdisciplinary team meetings to discuss patient care strategies and contributing to total treatment planning.Instructing patients and their families on the use of therapy devices and effective home exercises to promote rehabilitation outside of clinical sessions.Staying current with best practices and changes in regulations impacting physical therapy in both clinical and home care settings.Patient Population / Client FocusThe ideal candidate will work with a diverse patient population ranging from post-operative orthopedic patients to individuals managing chronic conditions. Our patients often benefit from tailored therapy that addresses specific challenges encountered in their home environments, a key area of focus in our individualized care plans.QualificationsTo succeed in this role, candidates must possess the following qualifications:A Bachelor's Degree in Physical Therapy from an accredited program.Current licensure as a physical therapist in New Jersey.At least one year of clinical experience in acute care or home care settings, with a clear understanding of orthopedic rehabilitation principles.Current CPR certification, and valid driver's license with automobile insurance.Strong communication skills to effectively engage with patients, families, and the interdisciplinary team.A proactive approach to problem-solving and the ability to work independently while managing patient care.Preferred ExperienceWhile not required, the following attributes will enhance a candidate's application:Experience working in home healthcare or rehabilitation settings with a focus on orthopedic cases.A Doctorate in Physical Therapy (DPT) is preferred, demonstrating advanced knowledge of clinical practice.Proficiency in utilizing electronic health record (EHR) systems for documentation and patient tracking.A professional commitment to continuing education and staying updated with evidence-based practices in physical therapy.Compensation and BenefitsSpring Leaf Rehabilitation Center offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
