Work Environment

This position is focused on home care, providing you with the unique opportunity to work in patients' homes, delivering personalized therapy that adapts to their living environments. You will typically work Monday to Friday from 8:00 am to 5:00 pm, allowing for a balanced work-life schedule. As an integral part of our mobile healthcare team, you will collaborate with colleagues to ensure every patient receives high-quality, evidence-based treatments.

Key Responsibilities

• Conduct comprehensive evaluations to assess patients' functional capabilities and limitations, ensuring individualized treatment plans are developed.
• Implement evidence-based therapeutic modalities and interventions that promote healing and recovery, while continually adjusting these to match patients' evolving needs.
• Maintain detailed documentation of all patient interactions, treatment plans, progress notes, and discharge summaries to ensure seamless transitions of care and regulatory compliance.
• Work collaboratively with other healthcare professionals, ensuring that all aspects of patient well-being are addressed through coordinated care efforts.
• Educate patients and their families on therapeutic exercises, home modifications, and wellness strategies to encourage ongoing improvement and lifelong mobility.
• Participate actively in team meetings and continual professional development sessions to enhance your skills and contribute to our shared knowledge base.
• Stay current with state laws and practice regulations to ensure compliance in all operational aspects of physical therapy delivery.

Patient Population / Client Focus

You will be working predominantly with orthopedic patients, including those recovering from surgeries, injuries, and chronic pain conditions. Our approach emphasizes individualized care tailored to the specific needs of each patient, ensuring that treatment is not only effective but also holistic and compassionate.

Qualifications

• A Doctorate of Physical Therapy (DPT) from an accredited institution is required.
• A valid physical therapy state license to practice in Florida must be actively maintained.
• At least 2 years of relevant experience in orthopedic physical therapy is necessary, with a proven track record of developing successful treatment plans.
• Current CPR certification is mandatory for ensuring patient safety.
• Exceptional interpersonal skills and the ability to build rapport with patients and their families quickly.

Preferred Experience

Compensation and Benefits
