Key Responsibilities\n

• Conduct comprehensive assessments and ongoing evaluations to determine the needs of each patient, utilizing standardized tools and frameworks.

• Develop and share treatment plans with supervisors detailing appointment schedules, home visit logistics, and progress tracking methods.

• Organize and conduct home visits as necessary for in-depth observations, chart reviews, and collaborative discussions with other healthcare personnel.

• Actively participate in interdisciplinary team meetings, contributing expertise to refine therapeutic approaches and improve care delivery.

• Document monthly and quarterly progress reports that capture essential developments and milestones in patient care.

• Coordinate training sessions and workshops aimed at enhancing team competencies in implementing care plans and managing documentation effectively.

Qualifications\n

• A Master’s Degree or Doctorate in Counseling, Psychology, or a related field is required.

• Possession of an active and unrestricted LPC license in the state of Georgia.
