This position is a remote role, utilizing a telehealth model to connect with patients. Key Responsibilities\n

• Collaborate with primary care providers to identify, assess, and manage mental health and psychosocial issues, actively participating as a member of the primary care team.

• Conduct thorough patient intakes that include diagnostic and functional evaluations, making informed recommendations for treatment objectives and plans.
\n
• Implement brief, focused interventions for clients in need of immediate mental health support, tailored to each patient's unique circumstances and needs.
\n
• Provide ongoing follow-ups to monitor patient progress, adjusting treatment strategies as necessary to ensure optimal outcomes.
\n
• Develop and implement relapse prevention plans where appropriate, aiding patients in sustaining stability in their mental health.
\n
• Identify at-risk patients and create proactive care plans to mitigate the deterioration of their conditions.
\n
• Coordinate with other healthcare providers to ensure comprehensive behavioral health care, with patient consent, integrating outside resources when necessary.

• Maintain detailed documentation of patient progress in compliance with clinic protocols and treatment charting standards.
\n
• Collaborate with primary care providers to formulate effective treatment plans while keeping them informed on patient needs and advancements.

• Teach patients self-management skills and provide educational resources, empowering them to take an active role in their recovery process.

• Offer consultative support to clinic management and team members regarding behavioral health services and program evaluations.

Patient Population / Client Focus\n

This counseling position serves a diverse range of patients, encompassing individuals from various backgrounds with differing mental health needs. You will work with clients facing challenges such as anxiety, depression, trauma, and stress-related issues, providing them with the necessary tools to improve their mental health and quality of life.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling is required.

• Must possess a current License as a Professional Counselor (LPC).

• Thorough understanding of mental health assessments, treatment planning, and various therapeutic modalities, including family therapy and psychotherapeutic techniques.

• Proficiency in assessing individual needs and effectively evaluating the care required.
\n
• Demonstrated ability to establish productive relationships with patients, their families, and other professionals within the healthcare environment.

• Strong communication skills, both oral and written, to clearly articulate ideas and patient progress.

Preferred Experience\n

• Bi-lingual skills in English and Spanish are highly desirable, but not mandatory.

• Experience in working within a Telehealth setting or familiarity with tele-therapy modalities.

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 to 105,000 annually, commensurate with experience. Additional benefits include a sign-on bonus, health insurance, retirement options, paid time off, and continued professional development opportunities.
